Maybe that also explains Jack seeing his Dad all over the place...

Maybe Eko fears that he lost his soul, and that's what the black cloud was - his soul.
Id's love to get a closer look at the people in the clouds to see if they were people from his flashbacks, or people that died on the island.

The smoke monster had images of Eko as a kid, the guy he shot, a church and him holding his shot brother inside it.
